What is transition planning?

The goals of the transition planning process are to:​
 

  • Incorporate a student’s needs, abilities, interests, and strengths into planning for postsecondary activities, including college and career readiness
     

  • Enable each student and their family to make informed choices
     

  • Include community experiences in school instruction
     

  • Develop a student’s employment and adult living skills
     

  • Enable each student to find success in moving from school to post-school activities

1:1 Coaching

Our brains are like muscles. Without practice, they weaken. Do not let your student fall behind once they graduate.

What is 1:1 coaching?

Your student will meet 1-to-1 with an employment specialist to provide personalized guidance and support to help them clarify their career goals, identify strengths and areas for development, explore career options, and create a plan to achieve their professional objectives. 

 

These sessions will involve discussing career interests, skills, values, and aspirations, as well as providing feedback, advice, and resources to help the student make informed decisions about their career path. You can meet 1:1 either virtually or in-person.

Virtual Coaching

Virtual coaching sessions offer a convenient and flexible way to receive personalized guidance and support from our experienced specialists. These sessions provide a safe and supportive environment to help your student make informed decisions about their career journey.

 

Experience the benefits of virtual coaching to boost your confidence and progress towards your goals from the comfort of your own space.

In-Person Coaching

In-person coaching sessions provide a hands-on and immersive experience where you can engage with our experienced specialists face-to-face.

 

Students can build a deeper connection with their coach, offering an environment for open and honest conversations about your goals and aspirations. These sessions allow for real-time interaction, active participation, and personalized support to help you navigate your career path with clarity and confidence.

The Discovery Process

The discovery process involves activities that are designed to help identify consumer strengths, skills, interests, abilities, and capabilities. These activities aid in the identification of potential vocational themes.

Retention and Ongoing Support Services

In this stage, the focus is on providing as-needed supports to ensure long-term retention. This may include onsite job coaching, ongoing assistance, training, or other resources to help the individual maintain employment successfully.​

Job Development & Placement

In the job development and placement stage, the focus is on researching available opportunities, applying for jobs, assistance with resume and/or interviewing skills, and assisting with job placement.

The Discovery Process

With Discovery Process, all the people who care about you team up to focus on you in finding out more in detail who you are, what are your strengths to search for a job designed just for you, they concentrate in your abilities and interests and support you when planning or your possible career options that match with your needs, preferences and goals.

 

It is an individualized experience where you get to meet a specialized person that will help you learn more about yourself in what you are good at not only at school but also at home and in your community so you can reach your goals better and be successful by reinforcing or building new skills at home or when you work in your community.

      and activities: When are they at their best?

    • Current skills and levels of performance.

    • Effective strategies, ideal working conditions also including those environments and strategies that should be avoided.

Job Development and Placement 

Career Awareness

  • Exposure to why people need and want to work; contributions to lifestyle and life choices.​

  • Exposure to a wide range of careers and jobs.

  • Exposure to work expectations, education/training skill requirements, and social/communication skills.

Career Preparation

  • Narrowing and confirming one career field; students may still explore a range of positions within this field.

  • Increasing work experience opportunities to prepare for full-time work in the community

  • Refine interviewing and job seeking skills for work experience positions and develop job

  • maintenance skills and social/communication skills to keep one’s position.

  • Identify potential career advancement and self-advocacy opportunities and strategies.

Career Exploration

  • Job shadowing and trial work experiences to narrow career choices to 1 or 2 preferred careers.

  • Matching career requirements with personal abilities, skills, aptitudes, and preferred lifestyle; assessing careers for “best-match."

  • Developing a plan to meet requirements and achieve a long-term, successful career.

Career Assimilation

  • Implementation of job seeking, interviewing, and related skills to locate and obtain a job of choice, in a location of choice.

  • Ongoing match of work conditions and environment with personal lifestyle preferences and needs; implementation of natural supports; use of problem-solving skills and employment resources if needed.

  • Relationship management- participation in work culture and employee activities, on and off-the-job, to support satisfaction between career and worker.​

Retention and Ongoing Support Services

Job development, job retention and placement services

Facilitation of natural supports at the worksite

Regular observation or supervision of the individual via supervisor or in-person

Follow up services such as regular contact with the employers, supervisors and support staff

Reinforcing and stabilizing the job placement

Social skills training
